We no longer have time to call individual insurance companies to check coverage. Every massage appointment will only be for the allotted time and we will not be able to check individual coverage .
We ask that you please help us by having all of your insurance information ready at the time of appointment (see below for the information we need) or you will be asked to pay the full amount of the massage and we will provide you a receipt to claim on your insurance.

If you have questions or you would like help checking your insurance coverage send us an e-mail to admin@transconamassagetherapy with as much of the Direct Billing Form filled out as possible at least 3 days before your appointment and we will do what we can to help get everything set up for you!
If you are able to PRINT, FILL OUT and SIGN the consent forms before your appointment please do so. The PLAN MEMBER will need to sign, so if you are a dependant or a spouse please get the plan member to sign the forms for you. Please leave "provider" section blank as this will be for us to fill out.
